The network is also responsible for the implementation and execution of our managed care contracts, medical management services, quality improvement programs, data analysis, and information services.

Position Purpose:

Supports all aspects of the clinical members of the Care Management Team. Manages the flow of referrals and communications from the various sites across the Steward network and outside the network to all the Care Management programs. Serves as a key communication link between patients, referral sources, physicians, patient care facilities and Clinical Staff.

  • Daily creation of all Care Management cases in Care Management documentation system
  • Monitors ADT notifications and escalates any abnormalities

Telephone

  • Monitor SHCN Care Management phone line and follows up until issues are resolved
  • Perform targeted follow up calls for care management team

Faxes and mailings:

  • Distributes patient updates and Discharge Summaries received via the SHCN Fax line to the appropriate care management team member
  • Distributes care management correspondence to both patients and PCPs
  • Emails Rounder Scorecards to SNF rounders monthly

Administrative responsibilities

  • Verification of patient eligibility for care management programs
  • Provide information from electronic medical records of partners to care management team
  • Ad hoc reporting
  • Distribute skilled nursing facility scorecards on a regular basis
  • Distribute reports as requested by management team
  • Performs targeted follow up calls for care management staff
  • Documents within care management documentation system
  • Assist with meeting preparation and scheduling including preparation of meeting decks, insertion of data, ensure, font and colors are consistent
  • Assist with orientation schedule for new hires and prepares orientation materials
  • Performs other duties as assigned

Education / Experience / Other Requirements

Education:

  • Bachelor Science preferred

Years of Experience:

  • Minimum of 2 years of health care/customer service experience.

Work Related Experience:

  • Experience working with referral management in physician’s office or in health plans preferred
  • Experience working telephonically with customers and/or patients

Specialized Knowledge:

  • Electronic medical record experience preferred
  • Knowledge of health care concepts and medical terminology
  • Strong communication and interpersonal skills
  • Proficient with Microsoft Office programs
  • Ability to meet deadlines in fast paced environment
  • Demonstrates effective interpersonal skills in intense situations
  • Ability to multi task and work with multiple software applications
