Care Coordination

Works with Care Coordination Program Team, Local Performance team, and Network Physicians to ensure patient care is coordinated to improve quality, cost, and efficiency of care. Requires utilization of computer, web based programs, remote and face to face discussions with network Physicians and Practice staff, and coordination with Care Coordination and Local Chapter teams.

Local Chapter Medical Director

Leadership role in all aspects of Local Chapter Performance, including but not limited to Quality, Patient Experience and Total Medical Expense control. Collaborates with Regional and Local Accountable Care Organization and Management Services Organization, and Medical Leadership teams to ensure that adequate support is provided to the Local Chapter providers and practices. Reviews audits, utilization reports, patterns of practice and makes recommendations to correct problems identified.

Education / Experience / Other Requirements

Education:

· MD required

· Master's degree in business, technology or health care administration strongly preferred

Experience:

· Minimum of 5 years of experience in related roles

· Currently Practicing as a Primary Care Physician (Internal Medicine, Family Practice, Pediatrics

Specialized Knowledge:

· Working knowledge of Population Health, Accountable Care Organizations, Risk Contracts is highly desired
