Under the direction of the Network Diabetes and Nutrition Program Director, the SHCN RD, CDE, Local Program Coordinator, develops and directs the local Diabetes Care Program (DCP) to meet the requirements for program recognition by the American Diabetes Association and other regulatory agencies. Additional duties include but are not limited to:

  • Assisting with the annual planning and ongoing evaluation of the local DCP's resources and effectiveness for the Network Diabetes Advisory Committee
  • Acting as a liaison between the local program staff, Satellite DCP Medical Director, local facility Administration, Primary Care Clinicians, Network Diabetes Program Manager and Network Diabetes Advisory Committee
    Ensuring all DCP policy and procedures are adhered to
  • Submitting monthly statistics and local program activity reports
  • Assisting in the development and implementation of Network DCP CQI activities
  • Assisting in the engagement of patients in the risk management contracts
  • Distributing bi-annual patient satisfaction surveys
  • Scheduling diabetes support groups and community events each year
  • Assisting with the marketing of programs
  • Orientation and training of staff
  • Participating in Diabetes Educator meetings, Network Advisory Committee meetings and other meetings as needed
  • Performing other duties as needed
The SHCN RD, CDE, Local Program Coordinator coordinates the day-to-day operations of the DCP at the local facility. Duties include planning, adhering to and monitoring of: local program schedule; insurance referrals; billing issues; documentation in electronic medical record; and maintenance of the Network Diabetes Registry.
The SHCN RD, CDE, Local Program Coordinator provides 1:1 and group Diabetes Self-Management Education and Support (DSMES) to patients referred to the DCP per ADA Education Recognition Program standards, Network DCP policies and procedures, evidence-based guidelines and referring clinician's instructions. He/she documents assessment, treatment plan and follow-up of patients' progress in the medical record and provides written documentation summarizing the treatment plan and self-management training to the referring clinician. He/she collaborates with DCP staff to develop and deliver 6 diabetes support groups and 2 community events per year. He/she enters information on patients participating in the DSME cycle into the Network DCP Diabetes Registry.

The SHCN RD, CDE, Local Program Coordinator performs nutrition assessment, nutrition diagnosis, nutrition intervention, monitoring and evaluation per Academy of Nutrition and Dietetics evidence-based practice guidelines for patients referred for Medical Nutrition Therapy to the local DCP and other contracted entities.

He/she documents medical nutrition therapy assessment, treatment and follow-up of patient's nutritional status and management in the medical record.


Bachelor's degree in Nutrition or Dietetics. Master's degree preferred


· Certified as a Registered Dietitian with the Commission on Dietetic Registration

· Licensed as a Licensed Dietitian Nutritionist in the state of Massachusetts

· Must be a Certified Diabetes Educator or eligible to complete the CDE examination within the year

· Must maintain Basic Cardiac Life Support (BCLS) certification

Years of Experience:

· Minimum of 3-5 years relevant Registered Dietitian experience required

· A minimum of 3 years' experience providing Diabetes Self-Management Education and Medical Nutrition Therapy

Related Experience:

· Experience developing and delivering group education preferred

· Managerial experience preferred

· Knowledge of blood glucose meters, CGM's and insulin pumps preferred

· Microsoft Office Suite
