Under the direction of the Network Diabetes and Nutrition Program Director and Local Program Coordinator, the SHCN RN, CDE, provides 1:1 and group Diabetes Self-Management Education and Support (DSMES) to patients referred to the Diabetes Care Program per ADA Education Recognition Program standards, Network DCP policies and procedures, evidence-based guidelines and referring clinician's instructions. Upon the order of the authorized prescribing practitioner, the RN, CDE initiates insulin administration and/or administers insulin and/or titrates insulin per protocol. He/she inserts and trains patient on use of Continuous Glucose Monitors. The RN, CDE assesses readiness of patients who wish to initiate insulin pump therapy and educates and supports patients on insulin pumps. He/she documents assessment, treatment plan and follow-up of patients' progress in the medical record and provides written documentation summarizing the treatment plan and self-management training to the referring clinician. The RN, CDE enters information on patients participating in the DSME cycle into the Network DCP Diabetes Registry.

The RN, CDE collaborates with DCP staff to develop and deliver six diabetes support groups and two community events per year.

The RN, CDE participates in Diabetes Educator meetings, Network Diabetes Advisory Committee meetings, CQI activities and distribution of patient satisfaction surveys.

The RN, CDE performs other duties as needed.


· Graduate of accredited School of Nursing. BSN/MSN preferred.


· Current RN Licensure in MA.

· Must be a Certified Diabetes Educator or eligible to complete the CDE examination within the year.

· Must maintain Basic Cardiac Life Support (BCLS) certification.

Years of Experience:

· Minimum of 3-5 years' relevant clinical nursing experience required.

· A minimum of 3 years' experience providing Diabetes Self-Management Education.

Related Experience:

· Experience developing and delivering group education preferred

· Experience with blood glucose meters, CGM's and insulin pumps preferred

· Bilingual in Spanish or Portuguese preferred

· Microsoft Office Suite
